The George Washington University Hospital (GWUH) Heart Failure application was created in conjunction with the GWUH Cardiology Department. This app helps to keep recently discharged patients on track with their health. The app helps you to keep up with and stay on track of your medication, daily symptoms and important measures (like weight and blood pressure). It alerts your health care team of changes in your condition, so they can help you when you need it. The GWUH Heart app also provides daily educational tips to help you manage your condition, as well as an educational library.
