IF YOU DO NOT HAVE ELECTRONIC HEALTH RECORDS IN AN EHR SYSTEM, OR IF YOUR HEALTHCARE PROVIDER HAS NOT PROVIDED YOU WITH LOGIN CREDENTIALS TO ACCESS YOUR HEALTH RECORDS THROUGH THEIR WEB PORTAL, THIS APP WILL NOT FUNCTION FOR YOU.
