HOW DOES IT WORK?
1 / Get a prescription from your primary care doctor.
Salusive Health is only available through your healthcare provider. Your provider will give you a prescription for our program during your next visit if you qualify. You can then download our app for your iPhone device.
2 / One of our Salusive Health nurses will call you.
A friendly nurse will reach out to discuss your care plan between doctor visits, which may include a kit of health monitoring devices you’ll receive in the mail. Setup only takes a few minutes, and we’ll help you every step of the way.
3 / Download our app so you can share your vitals.
You can use our mobile app and health monitoring devices anywhere to collect your vitals, track your medication usage, and note any symptoms or questions for a Salusive Health nurse to review.
4 / We’ll monitor your health... so you can live your life.
Our nurses monitor your progress and pro-actively follow-up to answer questions and update your care plan. If anything of concern arises, we can alert your care provider.
Providers and insurers monitor the progress of their patients through a web-based portal in real time to prioritize follow-up for the patients that need it most.
