Description
HF Path is a self-management tool for Heart Failure patients that teaches them to track and manage their symptoms, weight and medication regularly, while also educating them to take small steps to improve their quality of life through engaging interactive courses. Users also have the opportunity to connect with others like them through the group chat function or through the AHA/ASA Support network. The tool can also be accessed via the web at www.heart.org/hfpath. HF Path also provides integration with Apple Health to allow tracking activity from numerous fitness devices.
