Humana CareMatch at Mac App Store analyse

The CareMatch app allows care managers affiliated with Senior Bridge/Humana Cares to acquire new patient’s right from their iPhone! Using the CareMatch app, affiliate care managers can register for, receive, and respond to referral alerts that are sent out when Humana members discharge from the hospital under the Senior Bridge/Humana Cares Transitions Program. Care managers can define the geographic area they wish to serve right from within the app, and will only receive referral alerts for members who are within that area. The first registered care manager to respond to a referral alert is granted the referral.
